Enrollment Fee: $150 (this is a one-time, non-refundable fee charged to your credit care after completing enrollment)

Monthly Membership: $149 (automatically charged monthly to your credit card until cancelled by you or T2 Health)

Membership is on a month-to-month basis and requires NO LONG-TERM CONTRACTS OR COMMITMENTS.

You can cancel at any time, for any reason, by sending us a message through the secure patient portal. And there are no cancellation fees! Your membership will terminate at the end of your current monthly term, after which you will no longer be charged your monthly membership fee.

It's that simple!

Basically, your membership does not include the following third-party and ancillary services (please see the Clinical Membership Agreement for official details):

Third-Party Services. Emergency care, hospital care, specialist care, third-party labs, or radiological studies (i.e. X-rays, MRI’s,), or any other third-party healthcare/medical services not personally provided by T2 Health or its’ physicians or staff.

Ancillary Services. Supplemental services personally ordered and/or provided by T2 Health or its’ Physicians, to support diagnosis and treatment of a Patient’s condition. These may include items such as laboratory fees, prescription medications, dietary supplements, medical equipment and supplies, shipping and/or handling fees associated with these services, and any diagnostic or treatment services that are not explicitly described in the Clinical Membership Agreement.

No, you must be a member to receive care at T2 Health.

However, we are happy to schedule you a free, no obligation, “Discovery Meeting” appointment. This provides you an opportunity to see our office, meet our Physicians and Staff, and answer any question you may have.

If we are not the "right fit” for you, that’s totally fine; the traditional model may be a better choice for you.  Later, if you decide you want to give us a try, we will be here waiting for you (granted there are openings, as we limit the number of patients in the practice).

Patients are always welcome to rejoin our family!

If a patient cancels their membership, they must wait for 3-months to rejoin and may be subject to, at the sole discretion of T2 Health, payment of the current enrollment fee as stated in the Membership Agreement at the time of re-enrollment.

Because we limit the number of patients to 500 per Personal Physician, we cannot guarantee there will be an opening if you wish to re-join at a later date.

We think yes, but it is not a requirement to become a Patient of T2 Health.

Direct Care is not an insurance plan nor a substitute for health insurance. We recommend patients continue to carry healthcare insurance and you should consult your health benefits advisor or agent to help you find a plan that works best with Direct Care and that will meet your specific needs.

Yes, however, T2 Health nor its Physicians participate in Medicare, therefore, your monthly membership fee and ALL other fees for services rendered CANNOT be submitted to Medicare for reimbursement.

Due to current federal laws (work is currently in progress to change this law), medications, labs, and diagnostic imaging cannot be submitted to Medicare. However, our membership discounted rates, depending on your specific coverage, are typically less expensive than going through Medicare.

Unfortunately, no. Your monthly membership fee is not yet defined as a "medical expense" in most states and, as such, is not deductible.

However, the American Academy of Private Physicians is lobbying hard to win rightful deductibility for these expenses. In the meanwhile, consult your tax consultant to clarify tax consequences in your particular circumstances.

In providing superb care with unprecedented sorts of access, we seek to prevent or reduce hospitalizations and specialty referrals.

In instances where those types of care are required, we will certainly continue to work with your hospitalist during your hospital stay as well as work closely with any specialist.

In some cases, depending on the level of care and the sorts of expertise required, we may rely on those specialized caregivers to manage your treatment.
